Fortis Enrollment Form

This year we are opening sections on Tuesday and Thursdays.  In order to open a section on Tuesday and Thursday, we require enough students to enroll to cover costs. There are four options on the enrollment form to choose from:

• Monday & Wednesday only – If you are only willing to enroll on Monday and Wednesday.  All grade levels will be offered on Monday & Wednesday.
• Tuesday & Thursday only – If you are only willing to enroll on Tuesday and Thursday.  Sections require minimum enrollment to be offered.
• Monday & Wednesday preferred – This only affects families after Monday & Wednesday sections are filled (after priority re-enrollment has finished).  If Monday and Wednesday classes are full, then students will be put into Tuesday & Thursday classes.
• Tuesday & Thursday preferred – Students will be placed in Tuesday & Thursday sections, and if those sections have not made before Monday & Wednesday sections have students desiring the last seats, parents will be given options to move to Monday & Wednesday section before we fill those seats with other students.

On the enrollment form there is also a section for notes on enrollment choice.  Some decisions may be contingent on siblings or other factors.  Please list those factors and we will work with each of those requirements individually in partnership with families.
